The Individualized Clinical Blueprint
This program provides a comprehensive medical evaluation, BMI analysis, and a personalized nutrition/lifestyle plan. If clinically indicated, your provider may send a prescription directly to the local retail pharmacy of your choice. This program covers the medical consult and prescription service only; medication costs are handled separately at your pharmacy.

The Metabolic Spark
Spark may be added to any eligible tier, such as Foundation Tier 2 + Spark, or listed as a standalone vitality boost when clinically appropriate. This add-on is designed to support cellular energy production and metabolic wellness. Final pricing and fit are confirmed during your visit.

Multi-Program Policy: One Visit, One Fee
If your clinical evaluation determines that you are a candidate for multiple programs, for example combining a Foundation Series weight loss plan with a Restore Series vitality plan, only one $60 consultation fee is required. Your single evaluation covers all treatments discussed during your visit.
Geographic Availability
Slice Telehealth Services LLC currently provides clinical services exclusively to residents of the State of Illinois. Patients must be physically located in Illinois at the time of their virtual visit. Plans to offer additional states may be added in the future.
Clinical Safety & Labs
To ensure your safety and monitor your progress, baseline laboratory testing is required for Restore, Momentus, and Foundation programs. Labs completed within the last 6 months may be reviewed, or a lab order may be provided for completion at a local facility. Lab review is included in the $60 consultation fee.
Pharmacy Fulfillment
Medications provided through Series packages are sourced from FDA-registered 503A or 503B compounding pharmacies. Patients may choose to use affiliated pharmacies for package pricing or have a prescription sent to a retail pharmacy of their choice through the Basic Series pathway.
Emergency Notice
Slice Telehealth is for non-emergency medical care only. If you are experiencing a medical emergency, such as chest pain, shortness of breath, severe allergic reaction, or another urgent medical concern, call 911 or go to the nearest emergency room immediately.
Refund Policy
The $60 clinical consultation fee is non-refundable once the appointment has been completed, as it covers the professional time and medical evaluation of the provider. If you need to reschedule, please do so at least 24 hours in advance. Package costs are non-refundable once the order has been submitted to the pharmacy.
